The City of Live Oak provides at no cost to the employee a Life and Accidental Death & Dismemberment (AD&D) Insurance benefit of $25,000. Employees can purchase supplemental life insurance for themselves, their spouse and/or children. 

Cost 

• Employee and City contributions amounts are subject to change in accordance with contract renewals. Employee contributions toward monthly premiums are determined by the applicable Memorandum of Understanding (MOU) for the specific bargaining unit.
